Annex A

Terms and Conditions

1. For VAT Suppliers: For NON-VAT Suppliers:

Less 5% withholding tax on VAT Less 3% withholding tax on NON-VAT

Plus 1% withholding tax on EVAT Plus 1% withholding tax on EVAT

2. Delivery period is within thirty (30) days from receipt of Purchase Order.

3. Place of delivery: Sogod Water District Office, Sogod, So. Leyte.

4. This Request for Quotation must be sealed and submitted or ready for pick up

on or before [date] 9:30 a.m.

ONLY SEALED QUOTATION WILL BE ACCEPTED.

5. If awarded the contract, payment shall be made after complete delivery and

final acceptance of the items procured.

6. Refusal to accept the Notice of Award maybe ground for imposition of

administrative sanctions under Rule XXIII of the Revised IRR of RA 9184.
